For MD/MS: additionally BAMS degree certificate, internship completion certificate, and AIAPGET scorecard. Fee by DD in favour of 'Principal, Shri Ayurved Mahavidyalaya, Nagpur' from a Nationalised Bank as per fee structure PDF.
