Step 1: Qualify NEET UG → Step 2: For AIQ (15%): Register on MCC (mcc.nic.in); For UP SQ (85%): Register on DMET UP portal (upneet.gov.in) → Step 3: Pay registration fee → Step 4: Fill choice preferences (select SGMCHRC Gorakhpur — Arogya Dham, Balapar Road, Sonbarsa, Gorakhpur — 273007) → Step 5: Seat allotment based on NEET rank + category + seats → Step 6: Download Provisional Allotment Letter from counselling portal → Step 7: Report to Nodal Center for document verification → Step 8: Report to SGMCHRC with documents → Step 9: Pay fees + complete admission formalities. Documents required: NEET UG Admit Card, NEET UG Scorecard, Class 10 Certificate + Marksheet, Class 12 Certificate + Marksheet, Preliminary Allotment Letter, Transfer Certificate (if applicable), Photo ID (Aadhaar/PAN), 8 passport-size photographs, PwD Certificate (if applicable). For NRI: NRI/OCI documents + Embassy Certificate.

New college (NMC LOP 03/10/2024). AY 2024-25: 100 seats. AY 2025-26: 150 seats. All via DMET UP UP NEET counselling. Compulsory rural service MANDATORY after MBBS — penalty if skipped. 15-day Deeksharambh orientation program for new students. Chancellor: Yogi Adityanath. Gorakhpur Airport: 40 km.
