Admission to BAMS is based entirely on NEET UG scores. State Quota seats (32) are filled through the Bihar Ayush Counselling conducted by the Bihar Combined Entrance Competitive Examination Board (CECEB). Central Quota / All India Quota seats (6) are managed by the Medical Counselling Committee (MCC) under the Ministry of AYUSH. Candidates must qualify NEET UG and participate in the respective counselling rounds.

As a government-run college under the Department of AYUSH, Bihar, the fee at GASACH Begusarai is state-regulated and very affordable. The annual BAMS fee is approximately ₹9,971 per year, making the total 4.5-year academic cost approximately ₹44,870. This is significantly lower than private Ayurveda colleges. The current intake capacity for BAMS at Govt. Ayodhya Shivkumari Ayurved College, Begusarai, is 38 seats per year — 6 seats under the Central (All India) Quota and 32 seats under the State Quota. Previously, the intake was 30 seats (session 2019-20), which was subsequently increased to 38.
